Client Contract

This contract is between Adele Swift, the therapist, and the client/s named below.  It sets out some fundamental agreements about the professional basis on which the therapist and client/s will meet or conduct on-line therapy. The work to which this contract relates is subject to English law. The therapist abides by the ethical framework as laid out by the governing body COSRT, and she will be happy to provide further details of this if you wish.

Confidentiality;

The content of the sessions will be held in the strictest confidence. The exceptions to this rule are as follows:

The therapist, in adherence to good practice, will discuss each of her clients with a fully qualified supervisor, who is a full member of, and abides by the ethical code of the relevant professional association stated above. However, the identity of the client will not be shared.

Should a client reveal to the therapist an intention of self-harm or harm to others, or disclose that he/she had committed a serious criminal offence or intends to do so; the therapist may contact the relevant public authority to inform them of this.

A client may wish the therapist to refer him/her to a specialist or to contact their GP on their behalf. If this is deemed to be appropriate the therapist will undertake to do this, with a client signed consent form.

If the therapist becomes incapacitated in any way and is unable to contact the client themselves, details of the client may be accessed by a therapeutic executor, who will contact the client to make arrangement regarding therapy. 

 

Notes & Personal Data;

The therapist will take brief notes of the session for her professional development and to support your therapy. These will be destroyed 6 years after the completion of the work. Notes and personal data will not be shared with other parties unless the client has agreed to this by signing a release form, or if  a legal order has been issued. 

The therapist will maintain registration with The Information Commissioner’s Office and will abide by current GDPR data protection laws.

 

Payment and Session Booking;

An initial deposit of 50% of the agreed fee will be payable on booking the first appointment. Payment of the agreed fee will then be made at the end of each session, with the deposit rolling over to the next week.  At the end of the final session the deposit will be credited towards the final session fee, or returned to the client if suitable notice has been given of termination of therapy and there is no final session.

48 hours’ notice should be given for cancellations; otherwise, clients will lose their deposit and will be expected to pay another deposit on booking the next session. 

 

Online Sessions: 

On-line sessions will run in a similar way to in-person sessions, time keeping will be adhered to, as will appropriate dress. 

The session should be carried out in a private space that cannot be interrupted by other household members or distractions such as pets.  The client may be instructed to alter lighting or positioning to assist with better communication and balance. 

In order to maintain security of confidentiality, sessions may not be recorded in any way.

 

Social Media:

Requests for friend contact through Facebook and similar will be declined, this policy is in place to maintain the professional relationship.

 

Duration of Sessions;

Each session will last for 50 minutes. Clients should usually attend weekly, this will be reviewed and adjusted as therapy progresses.  If a client decides to terminate therapy for any reason, at least one further session should be attended in order to finish the work undertaken in a suitable manner.

 

Therapeutic Relationship;

The therapist will use her training, experience, own intuition and understanding to help clients to resolve or come to terms with their difficulties.  The clients will undertake to work alongside the therapist in this joint endeavour. The clients and therapist will always speak to each other with respect. They will not raise their voices or threaten violence or try to intimidate in any way.
